c) The address of the entity\u2019s main establishment and any other legal establishments within the European Union or, if not established within the EU, the address of its appointed representative (as per Article 18, para. 3),
d) Updated contact details, including email addresses and telephone numbers of the entity and, where applicable, its representative (Article 18, para. 3),
e) The Member States in which the entity provides its services,
